What you need for your cancer risk assessment
To evaluate your risk for cancer, we need to learn more about your family history and your own medical history. You can provide this information as you complete our surveys and during your individual risk evaluation session. In order to develop the most accurate measure of your cancer risk and to provide the best plan to reduce your risk we need to collect the following information:
- Family history (A form is provided for you to collect information about your family members with and without cancer.)
- Medical history, especially for those family members diagnosed with cancer.

- Current cancer screening practices
- Current Medications: This includes all prescription medications, over-the-counter medications, alternative medications, and supplements.
- Radiology Tests: Reports from X-rays, CT scans, MRI scans, and mammography. Sometimes our physicians will request the actual films, either in hard copy or digital format, in addition to the written reports.
- Pathology Reports/Reviews: If you have had a biopsy or have been diagnosed with cancer, we will evaluate your reports. In certain cases we will ask for your pathology slides. Then a pathologist here at Fox Chase Cancer Center will confirm your diagnosis by reviewing your slides.
- Insurance ID Card: Please notify your insurance company prior to your visit. They may require a formal referral from your referring physician or they may give you a formal referral authorization. (Please note: The risk assessment visit, as well as the firm review and pathology review, if required, are services that will be billed to your insurance and are subject to your provider's policy limitations.
- A list of your questions.
