Please contact the following offices directly to request an appointment:
Call the New Patient Office at 888-FOX-CHASE or register online.
Please be prepared to supply the following information when you request a new patient appointment:
- Your name, address, telephone number and Social Security number
- Diagnosis and date of diagnosis
- How you received your diagnosis (i.e. CT scan, biopsy, etc.) and at what hospital or facility
- Name and contact information for your referring physician and/or primary-care physician (if you would like Fox Chase to send reports to your physician[s])

- Submit your contact, insurance, and medical history information over a secure connection. This shortens your first visit.
- A New Patient representative will call with a fax number for sending your medical records.
Once you have provided us your basic demographic information, insurance information, and medical history, our physicians will review your medical records.
This clinical assessment will ensure that your first scheduled visit is with the appropriate Fox Chase physician who can assist you with your clinical care needs.
Scheduling Your Appointment
After we receive the faxed copies of your medical reports and our physicians review them, the New Patient Office will call you.
The New Patient representative will match you with the most appropriate Fox Chase physician and help you schedule your first appointment as quickly as possible.
At this time, you will be asked for your insurance information, including the name of your insurance company, group number, ID number and for those with an HMO, your physician provider number. (If you have already submitted this information online, it will be reviewed for accuracy.)
Shortly after this call, you will receive in the mail Fox Chase Cancer Center's Patient Guide to Services, a complete packet of new patient information.
Pre-Registration: Insurance and Referrals
After your scheduling call from the New Patient representative, a Patient-Access staff member will contact you to verify your insurance information and discuss referrals, pre-certification, co-payments and financial counseling issues with you.
Confirming and Preparing for Your Visit
About 72 hours before your first visit, you will receive a confirmation call reminding you of your appointment.
In addition, you will receive a call from the Fox Chase resource nurse providing care in your disease-specific clinic. The nurse will do a brief assessment of your care-plan needs in preparation for your first visit.
What will happen on my first visit to Fox Chase?
Registration, consultation, and meeting your team.
On your first visit to Fox Chase, you will meet your cancer care team — experienced oncology specialists working together to develop an individualized treatment plan for you.
If you have not registered online, please arrive 45 minutes before your scheduled appointment that day for check-in and registration. Please allow two to three hours for your first visit.
